La césarienne à vif, une problématique complexe

Cette problématique s’adresse en premier lieu aux anesthésistes-réanimateurs de terrain, mais pas seulement car son analyse systémique montre clairement qu’elle concerne toute l’équipe.

Elle nous semble en tout cas être à l’origine d’un nombre croissant de contentieux et parait plus fréquente que l’on ne croit, à tel point qu’un numéro spécial de la revue « Douleur et analgésie » lui a été récemment et entièrement consacré (Douleur analg. (2016) 29 ; 61-122. DOI 10.1007/s11724-016-0451-6).

L’alarme a été donnée par une psychologue exerçant dans une Maternité lyonnaise qui est régulièrement confrontée à ces situations. Un groupe d’anesthésistes-réanimateurs, experts dans cette spécialité, a souhaité apporter sa contribution à l’analyse médicale de cette problématique complexe.

Un des articles de cette revue, cosigné par deux anesthésistes-réanimateurs médecins conseils pour des compagnies d’assurance (les Dr E. Lopard et H. Bouaziz) est consacré aux « Conséquences médicolégales des défauts d’anesthésie locorégionale pour césarienne ».

Une abondante littérature médicale permet d’estimer la fréquence des insuffisances d’anesthésie pour césarienne entre 0,1 % et plus de 10 % selon le type d’anesthésie locorégionale (ALR) les taux les plus élevés s’observant en cas d’analgésie péridurale complétée, et selon les conditions de réalisation de cette intervention.

An abundant medical literature estimates the frequency of anesthesia failure for caesarean section between 0.1% and more than 10%, depending on the type of locoregional anesthesia (ALR), the highest rates observed in case of analgesia epidural, and according to the conditions of realization of this intervention.
