OBJECTIF:
Le but de cette étude observationnelle était de décrire la gestion obstétricale dans les accouchements par extraction sous vide (EV) et de comparer ces résultats aux instructions des directives cliniques sur l’EV.

MÉTHODES:
En 2013, des données détaillées sur la gestion de 600 cas d’EV ont été collectées consécutivement auprès de six unités de livraison différentes en Suède. Chaque unité a également fourni ses propres recommandations cliniques en matière d’EV.

RÉSULTATS:
Au total, 93% des EV ont abouti à un accouchement vaginal, tandis que 7% ont échoué et ont été converties en une césarienne d’urgence. Dans 2,3% des cas, le temps d’extraction a dépassé 20 minutes et dans 6% plus de six tractions ont été utilisées pour administrer le fœtus. Le détachement de la coupe s’est produit dans 14,6% des cas, et 11% des livraisons ont fait l’objet d’expression abdominale. Dans 2,3% des cas, la position fœtale était supérieure au niveau des épines ischiatiques de la mère. Les directives cliniques sur l’EV variaient en termes de portée et de contenu entre les unités et étaient souvent incomplètes au regard des meilleures pratiques.

CONCLUSION:
La grande majorité des expulsions à l’aide de la ventouse ont été menées conformément aux recommandations de professionnelles. Cependant, dans quelques cas d’extractions, les règles de sécurité ont été ignorées et plus de six tentatives ou une durée d’extraction de plus de 20 minutes ont été utilisées pour effectuer la naissance.

Abstract (English)  :

OBJECTIVE:
The aim of this observational study was to describe the obstetric management in vacuum extraction (VE) deliveries and to compare these findings to instructions in clinical guidelines on VE.

METHODS:
In 2013, detailed data on management of 600 VE cases were consecutively collected from six different delivery units in Sweden. Each unit also contributed their own clinical VE guideline.

RESULTS:
In total, 93% of the VEs ended with a vaginal delivery while 7% failed and were converted to an emergency cesarean section. In 2.3% extraction time exceeded 20 minutes, and in 6% more than six pulls were used to deliver the fetus. Cup detachment occurred in 14.6%, and fundal pressure was used in 11% of the deliveries. In 2.3%, fetal station was assessed as above the level of the maternal ischial spines. The clinical guidelines on VE varied in scope and content between units, and were often incomplete according to best practice.
