L’expression abdominale se fait manuellement avec les avant-bras, les coudes ou les paumes du praticien pressés sur le fond utérin à un angle de 30 à 45 ° par rapport à la colonne vertébrale maternelle dans la direction du bassin. La pression est appliquée dans la direction longitudinale. Une pression douce, ferme et régulière est recommandée. Les différents effets secondaires de l’application de l’expression abdominale comprennent : rupture utérine, rétention urinaire post-partum, traumatisme périnéal sévère et douleur, fracture des côtes, dyspareunie post-partum et détresse ou traumatisme néonatal potentiel. Par conséquent, la prudence est recommandée dans l’application de l’expression abdominale. Ainsi, seuls le personnel médical ou les infirmiers bien formés et expérimentés devraient être autorisés à pratiquer cette intervention.

Abstract (English)  :

Women who undergo vaginal birth apply uterine fundal pressure during the second stage of labor. Uterine fundal pressure is done manually with the practitioner’s forearms, elbows, or palms pressed on the uterine fundus at a 30° to 45° angle to the maternal spine in the direction of the pelvis. The pressure is applied in the longitudinal direction. Gentle, firm, and steady pressure is recommended. The various side effects of applying uterine fundal pressure include: uterine rupture, postpartum urinary retention, severe perineal trauma and pain, rib fracture, postpartum dyspareunia, and potential neonatal distress or trauma. Therefore, caution is recommended in the application of uterine fundal pressure. Thus, only well-trained and experienced medical staff or nurses should be allowed to practice this intervention.
