Évaluer si Baby-guard, un nouveau dispositif médical doté d’une ceinture abdominale gonflable ergonomique à 3 chambres, peut réduire les complications associées à un accouchement par voie vaginale.

METHODES :
Une étude prospective randomisée et contrôlée menée auprès de 80 femmes enceintes en étude à l’aveugle ayant accouché à terme a été menée à l’hôpital San Giuseppe, à Empoli, en Italie. Dans le groupe d’étude (n = 40), la ceinture abdominale a été gonflée à des pressions thérapeutiques optimales. Dans le groupe témoin (n = 40), la ceinture abdominale a été gonflée à des pressions minimales non thérapeutiques. Les facteurs liés aux complications maternelles, fœtales et du travail lors de l’accouchement vaginal ont été évalués.

RÉSULTATS :
Comparativement au groupe témoin, les femmes du groupe d’étude ont présenté une incidence plus faible de lacérations périnéales et cervicales (P0,001) ; utilisation réduite de la manœuvre de Kristeller (P0.001) ; durée plus courte du deuxième stade du travail (P0.001) ; moins de fatigue psychologique et physique (P0,001) ; moins de demandes maternelles d’accouchement par césarienne pendant le travail (P0.001) ; moins d’extractions sous vide (P0,01) ; et moins d’accouchements par césarienne (P0.02). Aucune admission en unité de soins intensifs néonatals n’a été enregistrée dans le groupe d’étude contre 7 dans le groupe témoin (P0,012).

CONCLUSION :
L’utilisation du système de ceinture abdominale gonflable ergonomique à 3 chambres a permis de réduire l’incidence des risques liés au travail vaginal.

Abstract (English)  :

To evaluate whether Baby-guard-a new medical device with an ergonomic 3-chamber inflatable abdominal belt-can reduce complications associated with vaginal delivery.

METHODS:
A randomized controlled single blind prospective study of 80 pregnant women delivering at term was conducted at San Giuseppe Hospital, Empoli, Italy. In the study group (n=40), the abdominal belt was inflated to optimal therapeutic pressures. In the control group (n=40), the abdominal belt was inflated to minimal, non-therapeutic pressures. Factors relating to maternal, fetal, and labor complications during vaginal delivery were evaluated.

RESULTS:
Compared with the control group, women in the study group experienced a lower incidence of perineal and cervical lacerations (P0.001); reduced use of the Kristeller maneuver (P0.001); shorter duration of the second stage of labor (P0.001); less psychologic and physical fatigue (P0.001); fewer maternal requests for cesarean delivery during labor (P0.001); fewer vacuum extractions (P0.01); and fewer cesarean deliveries (P0.02). No neonatal intensive care unit admissions were recorded in the study group versus 7 in the control group (P0.012).

CONCLUSION:
Use of the ergonomic 3-chamber inflatable abdominal belt system reduced the incidence of risks associated with vaginal labor
