Contexte

De plus en plus de femmes choisissent d’accoucher à la maison dans des pays disposant de ressources suffisantes. Des inquiétudes persistent quant au fait que les naissances à l’hôpital contribuent à une mortalité périnatale et néonatale plus élevée. Cette revue systématique et ces méta-analyses déterminent si le risque de perte fœtale ou néonatale diffère entre les femmes à faible risque qui accouchent avec l’intention d’accoucher à domicile par rapport aux femmes à faible risque ayant l’intention d’accoucher à l’hôpital.

Méthodes

En avril 2018, à partir de 1990, nous avons effectué une recherche dans cinq bases de données et utilisé R pour obtenir des estimations regroupées de l’effet. Nous avons stratifié par plan d’étude, paramètres d’étude et parité. Le résultat principal est tout décès périnatal ou néonatal après le début du travail. Le protocole de l’étude est évalué, publié et enregistré par les pairs (PROSPERO No.CRD42013004046).

Résultats

Nous avons identifié 14 études éligibles pour une méta-analyse, comprenant environ 500 000 naissances à domicile. Parmi les femmes nullipares qui souhaitent accoucher à domicile dans des environnements où les accoucheuses à domicile sont bien intégrées aux services de santé, le rapport de cotes (odds ratio) de la mortalité périnatale ou néonatale était de 1,07 (intervalle de confiance à 95% [CI] , 0,70 à 1,65) ; et dans des paramètres moins intégrés 3,17 (IC à 95%, 0,73 à 13,76). Parmi les femmes multipares souhaitant accoucher à domicile dans des environnements bien intégrés, le rapport estimé par rapport à celles envisageant d’accoucher à l’hôpital était de 1,08 (IC à 95%, 0,84 à 1,38) ; et dans les paramètres moins intégrés était de 1,58 (IC à 95%, 0,50 à 5,03).

Interprétation

Le risque de mortalité périnatale ou néonatale n’était pas différent lorsque la naissance était prévue à la maison ou à l’hôpital.

Abstract (English)  :

Background

More women are choosing to birth at home in well-resourced countries. Concerns persist that out-of-hospital birth contributes to higher perinatal and neonatal mortality. This systematic review and meta-analyses determines if risk of fetal or neonatal loss differs among low-risk women who begin labour intending to give birth at home compared to low-risk women intending to give birth in hospital.

Methods

In April 2018 we searched five databases from 1990 onward and used R to obtain pooled estimates of effect. We stratified by study design, study settings and parity. The primary outcome is any perinatal or neonatal death after the onset of labour. The study protocol is peer-reviewed, published and registered (PROSPERO No.CRD42013004046).

Findings

We identified 14 studies eligible for meta-analysis including ~ 500,000 intended home births. Among nulliparous women intending a home birth in settings where midwives attending home birth are well-integrated in health services, the odds ratio (OR) of perinatal or neonatal mortality compared to those intending hospital birth was 1.07 (95% Confidence Interval [CI] , 0.70 to 1.65); and in less integrated settings 3.17 (95% CI, 0.73 to 13.76). Among multiparous women intending a home birth in well-integrated settings, the estimated OR compared to those intending a hospital birth was 1.08 (95% CI, 0.84 to 1.38); and in less integrated settings was 1.58 (95% CI, 0.50 to 5.03).

Interpretation

The risk of perinatal or neonatal mortality was not different when birth was intended at home or in hospital.
