OBJECTIF :

Estimer la prévalence de l’état de stress post-traumatique (ESPT) après l’accouchement dans un groupe de femmes nigérianes en post-partum et examiner tous les facteurs associés.

CONCEPTION :

Une enquête transversale.

RÉGLAGE :

Cliniques postnatales et d’immunisation des nourrissons des cinq centres de santé du canton d’Ilesa, au Nigéria.

POPULATION :

Un total de 876 femmes 6 semaines après l’accouchement.

METHODES :

Les femmes en post-partum ont été évaluées pour le SSPT à 6 semaines. Les autres données recueillies portaient sur les caractéristiques démographiques, les détails de la grossesse et de l’accouchement et les résultats néonataux. En outre, les mesures suivantes ont été utilisées : l’interview neuropsychiatrique internationale MINI pour évaluer le SSPT, l’indice de satisfaction conjugale permettant de mesurer le degré de problème rencontré par un conjoint dans la relation conjugale, l’enquête sur le soutien social à la recherche sur les résultats médicaux Échelle des événements pour mesurer le stress de la vie au cours des 12 derniers mois et échelle de l’équipe de surveillance du travail qui mesure les expériences maternelles de contrôle pendant l’accouchement.

Principaux critères de jugement :

Prévalence du SSPT dans cette population de femmes nigérianes en post-partum et liens entre cette prévalence et d’autres caractéristiques maternelles et néonatales.

RÉSULTATS :

La prévalence du SSPT était de 5,9%. Les facteurs associés indépendamment au SSPT après l’accouchement incluent l’admission à l’hôpital pour complications de la grossesse (OR 11,86, IC 95% 6,36-22,10), l’accouchement par voie instrumentale (OR 7,94, IC 95% 3,96-16,15), la césarienne d’urgence (OR 7,31, 95% IC 3,53-15,10), retrait manuel du placenta (OR 4,96, IC à 95% 2,43-10,14) et mauvaise expérience de contrôle maternel pendant l’accouchement (OR 5,05, IC à 95% 2,69-9,48).

CONCLUSIONS :

La prévalence de l’ESPT après l’accouchement chez les femmes nigérianes est légèrement supérieure à celle observée dans la culture occidentale. Un modèle efficace de prévision du développement de l’ESPT après l’accouchement doit être développé et évalué, et les interventions visant à réduire l’incidence de l’ESPT après l’accouchement nécessitent des recherches plus poussées.

Abstract (English)  :

OBJECTIVE:

To estimate the prevalence of post-traumatic stress disorder (PTSD) after childbirth in a group of postpartum Nigerian women and to examine any associated factors.

DESIGN:

A cross-sectional survey.

SETTING:

Postnatal clinics and infant immunisation clinics of the five health centres in Ilesa Township, Nigeria.

POPULATION:

A total of 876 women at 6 weeks postpartum.

METHODS:

The postpartum women were assessed for PTSD at 6 weeks. Other data collected were demographic characteristics, details of pregnancy and delivery and neonatal outcome. Additionally, the following measures were used: the MINI International Neuropsychiatric Interview to assess PTSD, the Index of Marital Satisfaction to measure the degree of problem a spouse encounters in the marital relationship, the Medical Outcome Study Social Support Survey to measure social support, the Life Events Scale to measure the life stress covering the preceding 12 months and the Labour Agentry Scale that measures the maternal experiences of control during childbirth.

MAIN OUTCOME MEASURES:

Prevalence of PTSD in this population of postpartum Nigerian women, and how this prevalence related to other maternal and neonatal characteristics.

RESULTS:

The prevalence of PTSD was 5.9%. The factors independently associated with PTSD after childbirth include hospital admission due to pregnancy complications (OR 11.86, 95% CI 6.36-22.10), instrumental delivery (OR 7.94, 95% CI 3.91-16.15), emergency caesarean section (OR 7.31, 95% CI 3.53-15.10), manual removal of placenta (OR 4.96, 95% CI 2.43-10.14) and poor maternal experience of control during childbirth (OR 5.05, 95% CI 2.69-9.48).

CONCLUSIONS:

The prevalence of PTSD after childbirth in Nigerian women is slightly higher than those found in western culture. An effective model for the prediction of the development of PTSD after childbirth needs to be developed and evaluated, and interventions aimed at reducing the incidence of PTSD after childbirth need further research.

OBJETIVO:

Estimar a prevalência de transtorno de estresse pós-traumático (SPT) após o parto em um grupo de mulheres nigerianas pós-parto e examinar quaisquer fatores associados.

DESENHAR:

Uma pesquisa transversal.

CONFIGURAÇÃO:

Clínicas pós-natais e clínicas de imunização infantil dos cinco centros de saúde em Ilesa Township, Nigéria.

POPULAÇÃO:

Um total de 876 mulheres às 6 semanas após o parto.

MÉTODOS:

As puérperas foram avaliadas quanto ao SPT às 6 semanas. Outros dados coletados foram características demográficas, detalhes de gravidez e parto e resultado neonatal. Além disso, foram utilizadas as seguintes medidas: a Entrevista Neuropsiquiátrica Internacional do MINI para avaliar o SPT, o Índice de Satisfação Conjugal para medir o grau de problema que um cônjuge encontra na relação conjugal, a Pesquisa de Suporte Social do Medical Outcome Study para medir o suporte social, o Life Escala de Eventos para medir o estresse na vida dos 12 meses anteriores e a Escala de Agentes de Trabalho que mede as experiências maternas de controle durante o parto.

MEDIDAS DE SAÍDA PRINCIPAIS:

Prevalência de SPT nesta população de mulheres nigerianas no pós-parto e como essa prevalência se relacionou com outras características maternas e neonatais.

RESULTADOS:

A prevalência de SPT foi de 5,9%. Os fatores independentemente associados ao SPT após o parto incluem internação por complicações na gravidez (OR 11,86, IC 95% 6,36-22,10), parto instrumental (OR 7,94, IC 95% 3,91-16,15), cesariana de emergência (OR 7,31, 95% IC 3.53-15.10), remoção manual da placenta (OR 4,96, IC 95% 2,43-10,14) e pouca experiência materna de controle durante o parto (OR 5,05, IC 95% 2,69-9,48).

CONCLUSÕES:

A prevalência de SPT após o parto em mulheres nigerianas é um pouco maior do que a encontrada na cultura ocidental. Um modelo eficaz para a predição do desenvolvimento de SPT após o parto precisa ser desenvolvido e avaliado, e intervenções destinadas a reduzir a incidência de SPT após o parto precisam de mais pesquisas.
