Contexte : Un nombre croissant d’études ont récemment documenté le stress post-traumatique lié à l’expérience de l’accouchement à terme et à l’accouchement à l’accouchement. L’ampleur de cette affection et les facteurs susceptibles de mettre une femme à risque de développer des symptômes de trouble de stress post-traumatique post-partum (PTS-PTS) lié à l’accouchement ne sont pas entièrement compris.

Méthodes : Dans cette revue systématique de 36 articles représentant des études quantitatives d’échantillons principalement communautaires, nous avons examiné les taux de prévalence PP-SSPT et les prédicteurs associés en mettant l’accent sur le rôle du SSPT antérieur et le temps écoulé depuis l’accouchement.

Résultats : Une minorité significative de femmes a endossé le PP-PTSD après une naissance réussie. Les taux de PP-SSPT aigu se situaient entre 4,6 et 6,3%, et la confirmation de symptômes cliniquement significatifs de PP-SSPT a été identifiée chez jusqu’à 16,8% des femmes dans des échantillons communautaires d’études de haute qualité. Les expériences subjectives négatives d’accouchement sont devenues le prédicteur le plus important. L’approbation du SSPT avant l’accouchement a contribué au PP-SSPT ; Néanmoins, les femmes sans SSPT ont également présenté un SSPP-PP, avec des taux aigus de 4,6%, ce qui signifie un nouveau début de SSPT dans la période post-partum.

Abstract (English)  :

Background: Posttraumatic stress related with the childbirth experience of full-term delivery with health outcomes has been recently documented in a growing body of studies. The magnitude of this condition and the factors that might put a woman at risk for developing childbirth-related postpartum posttraumatic stress disorder (PP-PTSD) symptoms are not fully understood.

Methods: In this systematic review of 36 articles representing quantitative studies of primarily community samples, we set to examine PP-PTSD prevalence rates and associated predictors with a focus on the role of prior PTSD and time since childbirth.

Results: A significant minority of women endorsed PP-PTSD following successful birth. Acute PP-PTSD rates were between 4.6 and 6.3%, and endorsement of clinically significant PP-PTSD symptoms was identified in up to 16.8% of women in community samples of high quality studies. Negative subjective experience of childbirth emerged as the most important predictor. Endorsement of PTSD before childbirth contributed to PP-PTSD; nevertheless, women without PTSD also exhibited PP-PTSD, with acute rates at 4.6%, signifying a new PTSD onset in the postpartum period.

Antecedentes: O estresse pós-traumático relacionado à experiência do parto de parto a termo com resultados de saúde foi recentemente documentado em um crescente corpo de estudos. A magnitude dessa condição e os fatores que podem colocar uma mulher em risco de desenvolver sintomas de transtorno de estresse pós-traumático pós-parto (PP-SPT) não são totalmente compreendidos.

Métodos: Nesta revisão sistemática de 36 artigos representando estudos quantitativos principalmente de amostras da comunidade, propusemos examinar as taxas de prevalência de PP-SPT e preditores associados, com foco no papel do SPT prévio e no tempo desde o nascimento.

Resultados: Uma minoria significativa de mulheres endossou o PP-SPT após o nascimento bem-sucedido. As taxas agudas de PP-SPT foram entre 4,6 e 6,3%, e o endosso de sintomas clinicamente significativos de PP-SPT foi identificado em até 16,8% das mulheres em amostras comunitárias de estudos de alta qualidade. A experiência subjetiva negativa do parto surgiu como o preditor mais importante. O endosso do SPT antes do parto contribuiu para o SPT; no entanto, mulheres sem SPT também apresentaram SPT, com taxas agudas de 4,6%, significando um novo início de SPT no período pós-parto.

Bien que la majorité des femmes se débrouillent bien, l’accouchement peut être perçu comme une expérience très stressante et peut même entraîner l’apparition de symptômes de PP-PTSD. Des recherches supplémentaires sont nécessaires pour comprendre les conséquences de l’adaptation post-partum et du stress post-traumatique lié à l’accouchement.

Although the majority of women cope well, childbirth for some can be perceived as a highly stressful experience and even result in the development of PP-PTSD symptoms. More research is needed to understand postpartum adaption and childbirth-related posttraumatic stress outcomes.

Embora a maioria das mulheres lide bem, o parto de algumas pode ser percebido como uma experiência altamente estressante e até resultar no desenvolvimento de sintomas de SPT-PPT. Mais pesquisas são necessárias para entender a adaptação pós-parto e os resultados do estresse pós-traumático relacionado ao parto.
