CONTEXTE :
Dans le cadre de preuves très limitées, la présente étude visait à examiner de manière prospective l’impact des symptômes du trouble de stress post-traumatique (SPT) chez les mères après l’accouchement sur quatre domaines importants du développement de l’enfant : la motricité globale, la motricité fine, la communication et le développement socio-affectif.
MÉTHODE :
Cette étude fait partie de la grande cohorte de naissance d’Akershus. Les données des registres de naissance de l’hôpital ainsi que celles du questionnaire de 8 semaines et 2 ans post-partum ont été utilisées (n = 1472). Les domaines du développement de l’enfant qui étaient significativement corrélés aux symptômes du SPT ont été entrés dans les analyses de régression. Des analyses d’interaction ont été effectuées pour déterminer si l’influence des symptômes du SPT sur le développement de l’enfant était modérée par le sexe ou le tempérament du nourrisson.
RÉSULTATS :
Deux ans plus tard, les symptômes du SPT après l’accouchement avaient une relation prospective avec le développement socio-affectif de l’enfant pauvre. Cette relation est restée importante même en tenant compte des facteurs de confusion tels que la dépression et l’anxiété maternelles ou le tempérament du nourrisson. Le sexe et le tempérament du nourrisson ont tous deux modéré le lien entre les symptômes de stress post-traumatique chez la mère et son développement socio-affectif, c’est-à-dire que la charge de symptômes du syndrome de stress post-traumatique chez la mère augmentait.

Abstract (English)  :

BACKGROUND:
Against the background of very limited evidence, the present study aimed to prospectively examine the impact of maternal postpartum post-traumatic stress disorder (PTSD) symptoms on four important areas of child development, i.e. gross motor, fine motor, communication and social-emotional development.
METHOD:
This study is part of the large, population-based Akershus Birth Cohort. Data from the hospital’s birth record as well as questionnaire data from 8 weeks and 2 years postpartum were used (n = 1472). The domains of child development that were significantly correlated with PTSD symptoms were entered into regression analyses. Interaction analyses were run to test whether the influence of postpartum PTSD symptoms on child development was moderated by child sex or infant temperament.
RESULTS:
Postpartum PTSD symptoms had a prospective relationship with poor child social-emotional development 2 years later. This relationship remained significant even when adjusting for confounders such as maternal depression and anxiety or infant temperament. Both child sex and infant temperament moderated the association between maternal PTSD symptoms and child social-emotional development, i.e. with increasing maternal PTSD symptom load, boys and children with a difficult temperament were shown to have comparatively higher levels of social-emotional problems.

FUNDO:
Contra o pano de fundo de evidências muito limitadas, o presente estudo teve como objetivo examinar prospectivamente o impacto dos sintomas do transtorno de estresse pós-traumático pós-parto materno (SPT) em quatro áreas importantes do desenvolvimento infantil, como motor grosso, motor fino, comunicação e desenvolvimento socioemocional.
MÉTODO:
Este estudo faz parte da grande coorte de nascimentos Akershus, de base populacional. Foram utilizados dados do registro de nascimento do hospital e questionários de 8 semanas e 2 anos após o parto (n = 1472). Os domínios do desenvolvimento infantil que foram significativamente correlacionados com os sintomas de SPT foram inseridos nas análises de regressão. Análises de interação foram realizadas para testar se a influência dos sintomas pós-parto de SPT no desenvolvimento infantil foi moderada pelo sexo infantil ou pelo temperamento infantil.
RESULTADOS:
Os sintomas de SPT no pós-parto tiveram uma relação prospectiva com um desenvolvimento socioemocional infantil ruim 2 anos depois. Essa relação permaneceu significativa mesmo quando se ajustou para fatores de confusão, como depressão e ansiedade materna ou temperamento infantil. Tanto o sexo infantil quanto o temperamento infantil moderaram a associação entre os sintomas de SPT materno e o desenvolvimento sócio-emocional da criança, ou seja, com o aumento da carga de sintomas de SPT materno, meninos e crianças com um temperamento difícil mostraram níveis comparativamente mais altos de problemas socioemocionais.

Des études complémentaires sont nécessaires pour approfondir les mécanismes à l’œuvre.
