Objectif :
Comparer les trajectoires et déterminer les facteurs prédictifs de la détresse maternelle définis comme un spectre continu de symptomatologie et de symptomatologie élevée, de dépression, d’anxiété et de stress post-traumatique spécifique à la périnatalité chez les mères de très faible poids de naissance hospitalisées en unité de soins intensifs néonataux (USIN).
Méthode :
Soixante-neuf mères ont rempli des questionnaires psychologiques au cours du premier mois suivant l’hospitalisation de leur enfant dans l’USIN et à nouveau 2 semaines avant le congé de l’USIN. Les modèles de régression multiple ont déterminé les prédicteurs de détresse maternelle psychologiques, reproductifs, sociodémographiques et médicaux chez les nourrissons.
Résultats :
Le stress post-traumatique spécifique à la périnatalité est resté stable tout au long de l’hospitalisation à l’USIN, alors que les autres aspects de la détresse ont diminué. Les antécédents psychologiques et les variables médicales relatives au nourrisson prédisaient un PPTS plus élevé mais aucun autre aspect de la détresse Les variables de reproduction prédisaient l’anxiété et les TSPP ; Des antécédents de perte fœtale prédisaient initialement une PPTS plus faible, mais tout au long de l’hospitalisation, le statut de primipara est apparu comme un facteur prédictif d’une anxiété et d’une PPTS plus élevées. Les variables sociodémographiques ont présidé à une détresse dépressive initiale, mais non ultérieure.

Abstract (English)  :

Objective:
To compare the trajectories and determine the predictors of maternal distress defined as a continuous spectrum of symptomatology and elevated symptomatology, of depression, anxiety, and perinatal-specific posttraumatic stress (PPTS), in mothers of very low birth weight (VLBW) infants throughout the neonatal intensive care unit (NICU) hospitalization.
Method:
Sixty-nine mothers completed psychological questionnaires within the first month of their infant’s NICU hospitalization and again 2 weeks before NICU discharge. Multiple regression models determined maternal psychological, reproductive, sociodemographic, and infant medical predictors of maternal distress.
Results:
Perinatal-specific posttraumatic stress remained stable throughout the NICU hospitalization, whereas other aspects of distress declined. Previous psychological history and infant medical variables predicted higher PPTS but no other aspects of distress. Reproductive variables predicted anxiety and PPTS; history of fetal loss initially predicted lower PPTS but throughout hospitalization primipara status emerged as a predictor of higher anxiety and PPTS. Sociodemographic variables predicated initial, but not later, depressive distress.

Objetivo:
Comparar as trajetórias e preditores de sofrimento materno definidos como um espectro contínuo de sintomatologia e sintomatologia elevada, de depressão, ansiedade e estresse pós-traumático específico perinatal (PPTS), em mães de bebês com baixo peso ao nascer (MBPP) em todo o internação em unidade de terapia intensiva neonatal (UTIN).
Método:
Sessenta e nove mães completaram questionários psicológicos no primeiro mês após a internação na UTIN do bebê e novamente duas semanas antes da alta da UTIN. Vários modelos de regressão determinaram preditores médicos psicológicos, reprodutivos, sociodemográficos e médicos maternos de sofrimento materno.
Resultados:
O estresse pós-traumático específico perinatal permaneceu estável durante toda a internação na UTIN, e outros aspectos do sofrimento diminuíram. História psicológica prévia e outras variáveis ​​médicas PPTS, mas nenhum outro aspecto do sofrimento. Variáveis ​​reprodutivas previram ansiedade e PPTS; a história de perda fetal previu primeiramente que o objetivo mais baixo de PPTS do status de hospitalização primípara emergiu como preditor de maior ansiedade e PPTS.
