Objectif : Le but de cette étude était de déterminer si deux séances de débriefing après un accouchement opératoire pouvaient réduire la peur d’une femme pour un futur accouchement.
Conception : Essai contrôlé randomisé prospectif (ECR) avec deux bras comparant le débriefing, visant à réduire la peur d’un futur accouchement, avec des soins standard après la naissance.
Cadre : Hôpital général de district avec 2500 accouchements par an.
Échantillon : Trois cent dix-neuf mères qui ont accouché d’un premier enfant par accouchement chirurgical (p. ex. forceps, aspirateur ou césarienne d’urgence). L’étude a eu lieu à Huddersfield Royal Infirmary, de janvier 2002 à juillet 2003.
Méthodes : Débriefing par des sages-femmes communautaires spécifiquement formées au débriefing post-partum à 10 jours et 10 semaines.
Principal critère de jugement : la crainte de l’accouchement a été évaluée à l’aide de la Wijma Delivery Expectancy Scale (WDEQ). Les scores WDEQ ont été mesurés 10 jours, 10 semaines et 20 semaines après l’accouchement.
Résultats : La crainte de l’accouchement mesurée par le WDEQ a été plus faible tout au long de l’étude pour le groupe de débriefing. Cependant, elle n’a jamais atteint une signification statistique à court terme [débriefing 10 jours ¼ 94,5, contrôle ¼ 97,5 (P ¼ 0,295), Débriefing 10 semaines¼92,0, contrôle¼97,9 (P¼0,076), 20 semaines débriefing¼90,9, contrôle¼97,4 (P¼0,057)] .

Abstract (English)  :

Objective: The aim of this study was to determine if two debriefing sessions following an operative deliverycould reduce a woman’s fear of future childbirth.
Design: Prospective randomised controlled trial (RCT) with two arms comparing debriefing, aimed to reducefear of future childbirth, with standard care after birth.
Setting: District General Hospital with 2500 deliveries per year.
Sample: Three hundred and nineteen mothers who delivered a first child by operative delivery (i.e. forceps,vacuum or emergency caesarean section).
The study took place at Huddersfield Royal Infirmary, fromJanuary 2002 to July 2003.
Methods: Debriefing by community midwives specifically trained in postpartum debriefing at 10 days and10 weeks.
Main outcome measure: Fear of childbirth was assessed using the Wijma Delivery Expectancy Scale (WDEQ). WDEQ scores were measured 10 days, 10 weeks and 20 weeks following delivery.
Results: Fear of childbirth as measured by the WDEQ was lower throughout the study for the debriefing group.However, it never reached statistical significance in the short term [10 days debriefing¼94.5, control¼97.5 (P¼0.295), 10 weeks debriefing¼92.0, control¼97.9 (P¼0.076), 20 weeks debriefing¼90.9,control¼97.4 (P¼0.057)] .

Cette étude montre qu’à court terme, il n’y a pas de différence significative dans les scores de peur de l’accouchement du WDEQ. Le groupe de débriefing montre une tendance à des scores inférieurs. Un suivi à long terme de ces cas peut être plus pertinent.
