Contexte : La recherche axée sur la santé mentale paternelle est limitée et les conséquences de cette situation sont ignorées. Par exemple, on sait peu de choses sur les expériences des hommes qui assistent à un accouchement traumatisant pour leur partenaire et à l’impact qui en résulte. Par conséquent, le but de cette étude était d’explorer les expériences des hommes en tant que témoins d’un accouchement traumatique, comment ces expériences ont eu un impact sur leur bien-être et quel soutien ils ont reçu pendant et après l’accouchement traumatique.

Méthodes : Soixante et un participants ont été recrutés via les médias sociaux ciblés pour remplir un questionnaire qualitatif en ligne concernant leur expérience de traumatisme à la naissance. Les participants éligibles étaient âgés de plus de dix-huit ans, résidaient au Royaume-Uni et avaient été témoins d’un accouchement traumatisant qui n’a pas entraîné de décès. Une analyse thématique a été utilisée pour analyser les données du questionnaire.

Résultats : Trois thèmes principaux ont été identifiés : « la compréhension des pères de l’expérience » (sous-thèmes : rien ne peut vous y préparer ; simplement un passager ; des expériences mitigées avec le personnel ; pas à propos de moi) ; « Traumatisme de la vie après la naissance » (sous-thèmes : virilité après la naissance ; incapacité à être heureux ; impact sur les relations) ; et « le soutien que les pères ont reçu par rapport à ce qu’ils voulaient » (sous-thèmes : soutien prénatal, soutien à la naissance et soutien postnatal).

Conclusions : les pères ont rapporté des expériences significatives lors de l’accouchement lorsqu’ils avaient été témoins de l’accouchement traumatisant de leur partenaire. Ils estimaient que cela avait un impact sur leur santé mentale et leurs relations pendant la période postnatale. Cependant, il n’y a pas de soutien reconnu au niveau national pour les pères à utiliser à la suite de leurs expériences. Les participants ont attribué cela au fait qu’ils étaient perçus comme moins importants que les femmes pendant la période périnatale / d’éducation des enfants, et aux perceptions des services de maternité à l’égard du père en général. Cela implique notamment d’assurer un soutien à toutes les personnes impliquées dans la période périnatale, avec une formation supplémentaire du personnel orientée vers le rôle du père.

Abstract (English)  :

Background: Research focusing on paternal mental health is limited and the consequences of this are ignored. For example, little is known about the experiences of men who witness their partner’s traumatic birth and the subsequent impact. Therefore, the aim of this study was to explore men’s experiences of witnessing a traumatic birth, how these experiences impacted on their wellbeing, and what support they received during and following the traumatic birth.

Methods: Sixty-one participants were recruited via targeted social media to complete an online qualitative questionnaire regarding their birth trauma experience. Eligible participants were aged over eighteen, resided in the UK and had witnessed a traumatic birth that did not result in loss of life. Thematic analysis was used to analyse the questionnaire data.

Results: Three main themes were identified: ‘fathers’ understanding of the experience’ (subthemes: nothing can prepare you for it; merely a passenger; mixed experiences with staff; not about me); ‘life after birth trauma’ (subthemes: manhood after birth; inability to be happy; impact on relationships); and ‘the support fathers received vs what they wanted’ (subthemes: prenatal support; birth support; and postnatal support).

Conclusions: Fathers reported significant experiences during childbirth when they had witnessed their partner’s traumatic birth. They felt this impacted on their mental health and relationships long into the postnatal period. However, there is no nationally recognised support in place for fathers to use as a result of their experiences. The participants attributed this to being perceived as less important than women in the perinatal/childrearing period, and maternity services’ perceptions of the father more generally. Implications include ensuring support is given to all involved in the perinatal period, with additional staff training geared towards the father’s role.

Antecedentes: A pesquisa focada na saúde mental paterna é limitada e as consequências disso são ignoradas. Por exemplo, pouco se sabe sobre as experiências de homens que testemunham o nascimento traumático de seu parceiro e o impacto subsequente. Portanto, o objetivo deste estudo foi explorar as experiências dos homens de testemunhar um parto traumático, como essas experiências impactaram seu bem-estar e que apoio receberam durante e após o nascimento traumático.

Métodos: Sessenta e um participantes foram recrutados por meio de mídias sociais direcionadas para preencher um questionário qualitativo on-line sobre sua experiência de trauma no nascimento. Os participantes elegíveis tinham mais de dezoito anos, residiam no Reino Unido e haviam testemunhado um nascimento traumático que não resultou em perda de vidas. A análise temática foi utilizada para analisar os dados do questionário.

Resultados: foram identificados três temas principais: ’compreensão dos pais sobre a experiência’ (subtemas: nada pode prepará-lo para isso; apenas um passageiro; experiências mistas com a equipe; não sobre mim); ’Vida após o trauma’ (subtemas: masculinidade após o nascimento; incapacidade de ser feliz; impacto nos relacionamentos); e “os pais de apoio recebidos versus o que desejavam“ (subtemas: suporte pré-natal; suporte ao nascimento; e suporte pós-natal).

Conclusões: Os pais relataram experiências significativas durante o parto quando testemunharam o nascimento traumático de seu parceiro. Eles sentiram isso impactado em sua saúde mental e relacionamentos durante o período pós-natal. No entanto, não existe um apoio reconhecido nacionalmente para os pais usarem como resultado de suas experiências. Os participantes atribuíram isso a ser percebido como menos importante do que as mulheres no período perinatal / infantil e as percepções dos serviços de maternidade sobre o pai em geral. As implicações incluem a garantia de apoio a todos os envolvidos no período perinatal, com treinamento adicional da equipe voltado para o papel do pai.
