CONTEXTE : En raison de la demande populaire pour un accouchement plus naturel, un nouveau concept a été introduit en 1991 dans notre clinique. Il consistait en une surveillance et une gestion de la naissance attentives, en un recours réduit aux méthodes invasives et au libre choix parmi les différentes méthodes de naissance, y compris la naissance dans l’eau et autres méthodes de naissance. Notre objectif était de déterminer l’influence de notre nouveau concept sur la manière dont les femmes accouchent et sur la gestion des naissances dans notre clinique.

MÉTHODES : Sur un total de 9 418 naissances entre 1991 et 1997 [nouveau concept de naissance KSF (KSF = Kantonsspital Frauenfeld)] , l’évolution de la structure des méthodes d’accouchement et de la gestion des naissances dans notre clinique sous l’influence du nouveau concept de naissance a été analysée. Les résultats ont été comparés à un groupe historique dans notre clinique, soit un total de 5 602 naissances de 1986 à 1990, et à des données provenant d’un groupe contemporain de cliniques suisses, à un total de 344 328 naissances de 1986 à 1997.

RÉSULTATS : Notre étude montre que les méthodes de naissance alternatives sont très populaires. Les taux de naissance dans l’eau ont augmenté régulièrement et se sont stabilisés autour de 40 à 50% des naissances spontanées. Les taux de natalité sur un tabouret d’accouchement Maia ont atteint un sommet de popularité en 1993 (23%) 5 ans après leur introduction, tombant à nouveau à 10% des naissances spontanées. Les taux d’accouchement se sont stabilisés autour de 40% des naissances spontanées. D’autres méthodes d’accouchement telles que la posture debout, soutenue par une corde, sur le tapis ou à quatre pattes sont beaucoup moins populaires. L’impact de notre nouveau concept de naissance sur différents aspects de la gestion des naissances varie considérablement d’un pays à l’autre. Le taux d’épisiotomie est passé d’un taux antérieur supérieur à 80% à un taux inférieur à 15%. Le taux de césariennes dans notre clinique (environ 10%) est resté nettement inférieur à la moyenne suisse (environ 15%). Le taux d’analgésie rachidienne et péridurale a été maintenu à environ 13%, tandis que le taux moyen en Suisse a doublé et atteint 23% en 1997. Les taux d’induction et d’amniotomie ainsi que l’utilisation d’ocytocine n’ont pas été influencés par le nouveau concept de naissance et sont comparables à la moyenne suisse.

CONCLUSION : Les méthodes de naissance alternatives, en particulier les naissances aquatiques, sont très recherchées. Cette pression populaire qui insiste sur une gestion des naissances moins invasive et plus naturelle peut être bien intégrée dans la pensée de la médecine classique axée sur la sécurité. Dans notre clinique, la tendance générale vers des mesures plus invasives dans la gestion des naissances pourrait être contrecarrée par l’introduction d’un nouveau concept d’accouchement utilisant des méthodes d’accouchement alternatives.

Abstract (English)  :

BACKGROUND: Because of popular demand for more natural childbirth, a new concept was introduced in 1991 in our clinic. It consisted of careful monitoring and birth management, restrictive use of invasive methods, and free choice of different birth methods including waterbirths and other alternative birth methods. Our aim was to determine the influence of our new birth concept on the way women give birth and on the birth management in our clinic.

METHODS: In a total of 9,418 births between 1991 and 1997 [new birth concept KSF (KSF = Kantonsspital Frauenfeld)] , the changing pattern of birth methods and birth management in our clinic under the influence of the new birth concept were analysed. The results were compared to a historical group in our clinic, a total of 5,602 births from 1986 to 1990, and to data from a contemporary group from Swiss clinics, a total of 344,328 births from 1986 to 1997.

FINDINGS: Our study shows that alternative birth methods are very popular. The waterbirth rates have risen steadily and stabilized at around 40-50% of the spontaneous births. The Maia-birthing stool births rates reached a peak of popularity in 1993 (23%) 5 years after their introduction, dropping again to 10% of the spontaneous births. The bedbirth rates have stabilized at around 40% of the spontaneous births. Other birth methods such as standing, supported by a rope, on the mat or on all fours are much less popular. The impact of our new birth concept on different aspects of birth management differs greatly from one to another. The episiotomy rate has dropped from a previous rate higher than 80% to a rate lower than 15%. The caesarean section rate in our clinic (around 10%) has remained substantially below the Swiss average (around 15%). The rate of the spinal and epidural analgesia was maintained at a constant level, around 13%, while the Swiss average rates doubled and reached 23% in 1997. The induction and amniotomy rates as well as the use of oxytocin were not influenced by the new birth concept and are comparable to the Swiss average.

CONCLUSION: Alternative birth methods and in particular waterbirths are very sought after. This popular pressure insisting upon less invasive, more natural birth management can be well integrated into the security-oriented way of thinking of classical medicine. In our clinic the general trend towards more invasive measures in birth management could be countered by the introduction of a new birth concept with alternative birth methods.
