L’accouchement par voie basse est associé à une mortalité et à une morbidité maternelle plus faibles que l’accouchement par césarienne avant travail (NP 2). Par rapport à la voie haute et à âge gestationnel égal, il diminue le risque de détresse respiratoire néonatale (NP3). Il expose en revanche à une augmentation du risque de lésions urinaires et anales dont les déterminants, les moyens de prévention et les conséquences à long terme sont encore mal connus (NP 2).

La césarienne en cours de travail est associée à un risque de mortalité et de morbidité maternelles nettement plus élevées que la césarienne avant travail.

Dans ces conditions, l’accouchement par voie basse doit être préféré a priori à la réalisation d’une césarienne avant travail, mais son bien fondé doit être reconsidéré dans toutes les situations où le risque de césarienne en cours de travail peut être affirmé comme très élevé a priori (NP 4). Cependant, le taux de césariennes pendant le travail à partir duquel une césarienne avant travail devrait être préférée à la tentative d’accouchement par voie basse n’est pas connu (NP 4).
