Les recommandations pour la pratique clinique sur le thème " Prévention anténatale du risque infectieux bactérien néonatal précoce " ont été élaborées à la demande de la Société Française de Pédiatrie (Groupe de Pathologie Infectieuse Pédiatrique).
Elles sont destinées aux gynécologues-obstétriciens, sages-femmes, bactériologistes, médecins généralistes, pédiatres néonatologues et réanimateurs pédiatriques.

Les propositions sont classées en grade A, B ou C selon les modalités suivantes :

* une recommandation de grade A est fondée sur une preuve scientifique établie par des études de fort niveau de preuve, par exemple essais comparatifs randomisés de forte puissance et sans biais majeur, méta-analyse d’essais contrôlés randomisés, analyse de décision basée sur des études bien menées ;
* une recommandation de grade B est fondée sur une présomption scientifique fournie par des études de niveau intermédiaire de preuve, par exemple essais comparatifs randomisés de faible puissance, études comparatives non randomisées bien menées, études de cohorte ;
* une recommandation de grade C est fondée sur des études de moindre niveau de preuve, par exemple études cas-témoins, séries de cas.

En l’absence de précision, les recommandations proposées correspondent à un accord professionnel.

La lutte contre les infections materno-fœtales constitue un enjeu majeur de santé publique en raison des séquelles néonatales, notamment neurologiques et pulmonaires, qu’elles peuvent engendrer.

Abstract (English)  :

The control of maternal and foetal infection is a major public health challenge given the consequences for the neonate, in particular neurological and pulmonary complications.

These clinical practice guidelines on "Antenatal Prevention of the risk of early neonatal bacterial infection" were produced at the request of the Paediatrics Infectious Disease group of the French Paediatrics Society (Société Française de Pédiatrie). They are intended for gyneacologists and obstetricians, midwives, microbiologists, general practitioners, paediatricians, neaonatologists and paediatric intensive care specialists.
