De plus en plus de femmes demandent à accoucher par césarienne en l’absence d’indication médicale, et les médecins hésitent sur la manière de répondre. Cette tendance est en partie due au fait qu’on perçoit la césarienne comme beaucoup plus sûre que dans le passé, et à la prise de conscience que la plupart des études sur les risques de la césarienne ont pu être biaisées, puisque les femmes qui ont des problèmes médicaux ou obstétricaux ont plus de chances d’être sélectionnées pour une césarienne programmée. Ainsi, la fréquence des mauvais résultats maternels ou néonataux pourrait être imputée au problème ayant entraîné la césarienne, plutôt qu’à la césarienne elle-même. La seule manière d’éviter ce biais de sélection serait de mener une enquête dans laquelle on dirigerait de manière aléatoire les femmes vers une césarienne programmée ou un accouchement vaginal. Quand on a fait cela, à l’occasion de l’essai randomisé sur l’accouchement à terme en siège, une enquête internationale à laquelle ont participé 2088 femmes dont le bébé se présentait en siège à terme, les risques de décès perinatal ou néonatal, ou de grave morbidité néonatale, étaient considérablement plus faibles dans le groupe césarienne, sans pour cela augmenter les risques de mort maternelle ou de morbidité maternelle grave.

Abstract (English)  :

A growing number of women are requesting delivery by elective cesarean section without an accepted "medical indication," and physicians are uncertain how to respond. This trend is due in part to the general perception that cesarean delivery is much safer now than in the past and to the recognition that most studies looking at the risks of cesarean section may have been biased, as women with medical or obstetric problems were more likely to have been selected for an elective cesarean section. Thus, the occurrence of poor maternal or neonatal outcomes may have been due to the problem necessitating the cesarean delivery rather than to the procedure itself. The only way to avoid this selection bias is to conduct a trial in which women would be randomly assigned to undergo a planned cesarean section or a planned vaginal birth. When this was done in the international randomized Term Breech Trial involving 2088 women with a singleton fetus in breech presentation at term, the risk of perinatal or neonatal death or of serious neonatal morbidity was significantly lower in the planned cesarean group, with no significant increase in the risk of maternal death or serious maternal morbidity.
