Cette revue discute des utilisations spécifiques et des avantages potentiels du forceps par rapport aux autres modes de délivrance. Pour permettre aux femmes de choisir en connaissance de cause leur mode d’accouchement, les obstétriciens doivent être correctement formés et supervisés pour l’utilisation du forceps.

Résumé des points

- La plupart des femmes visent un accouchement vaginal spontané

- Lorsque des complications surviennent au cours du deuxième stade du travail, il existe un choix entre un accouchement vaginal instrumental et une césarienne.

- Les obstétriciens ont de plus en plus recours à la césarienne lorsque des complications surviennent au deuxième stade du travail.

- Les lésions du plancher pelvien et les traumatismes du bébé sont plus fréquents après l’accouchement au forceps, mais les hémorragies maternelles majeures et la séparation du bébé sont plus fréquentes après la césarienne.

- Les femmes sont plus susceptibles d’accoucher spontanément par voie vaginale lors d’une grossesse ultérieure après un accouchement au forceps qu’après une césarienne.

Abstract (English)  :

This review discusses the specific uses and potential advantages of forceps over other modes of delivery. To enable women to make an informed choice about mode of delivery, obstetricians need to be adequately trained and supervised in the use of forceps.

- Most women aim for spontaneous vaginal delivery

- When complications arise in the second stage of labour there is a choice between instrumental vaginal delivery and caesarean section

- Obstetricians are increasingly choosing caesarean section when complications arise in the second stage of labour

- Injury to the pelvic floor and trauma to the baby are more common after forceps delivery, but major maternal haemorrhage and separation from the baby are more common after caesarean section

- Women are more likely to achieve a spontaneous vaginal delivery in a subsequent pregnancy after forceps delivery than after caesarean section
