Mesure télémétrique continue de la pression partielle en oxygène pendant l’accouchement

Introduction. Les positions verticales ou la déambulation pendant l’accouchement sont bonnes pour l’oxygénation fœtale. Pour étudier l’effet des positions d’accouchement sur l’oxygénation fœtale, il est nécessaire de permettre à la mère une totale liberté de mouvement. Il faut donc, au préalable de toute étude sur le sujet, disposer de capteurs permettant d’effectuer des mesures à distance sur le fœtus (capteurs télémétriques).
La télémétrie, si elle est techniquement réalisable, permettait de mesurer la pression partielle en oxygène du fœtus à l’aide de capteurs existants et sans restreindre la liberté de mouvement de la parturiente. Nous avons développé un système de télémétrie qui s’utilise avec un capteur de pression partielle en oxygène transcutanée du fœtus (ttcp02) et nous avons étudié les effets de la position et des changements de positions de la mère pendant un accouchement normal.

Méthode et matériel. Le système de surveillance est constitué de trois parties.
- Une unité de télémétrie avec le capteur ttcp02 qui transmet la mesure de la pression partielle en oxygène transcutanée (tcp02) et de la température
- un cardiotocogramme (CTG)
- un ordinateur pour stocker les résultats.

Toutes les données sont imprimées sur la sortie papier du CTG et transmises à un logiciel conçu spécialement pour afficher le rythme cardiaque fœtal, l’intensité des contractions utérines, la mesure de la pression partielle en oxygène transcutanée (ttcp02) et de la température. L’étude porte sur trois femmes qui ont adopté, au cours de leur accouchement, des positions classiques (décubitus dorsal ou position latérale), assises, verticales ou la déambulation, successivement et de façon aléatoire.

Résultats. Les positions et les changements de position ont un impact sur le rythme cardiaque fœtal, les contractions utérines, la pression partielle en oxygène transcutanée (ttcp02) et la température. Il semble y avoir une moindre oxygénation fœtale en position allongée sur le dos ou sur le côté. Les positions assises, debout et surtout la marche ont un effet positif.

Discussion. La télémétrie présente un intérêt pour étudier les éventuels intérêts cliniques des positions d’accouchement personnalisées.

Abstract (English)  :

INTRODUCTION: Upright or ambulatory birth positions are favorable for fetal oxygenation. Studies of fetal oxygenation with regard to maternal position require free maternal mobility. Therefore, telemetry for a fetal sensor for such investigations is a pre-requisite. Telemetry—if technically feasible—could enable monitoring of fetal oxygen partial pressure using an existing sensor without restricting the mobility of the parturient woman. We have developed a telemetry system for use with a fetal transcutaneous partial oxygen pressure sensor (ttcpO2) and have studied effects of maternal position and position changes during normal labor.

MATERIALS AND METHODS: The monitoring system consists of three parts: the telemetry unit with the ttcpO2 sensor to transmit the tcpO2 and the heating output telemetrically, a modified CTG monitor and a personal computer storing the measurements. All data were plotted on the CTG recording paper and fed into a new purpose-designed software, displaying fetal heart rate, the uterine contraction intensity, ttcpO2 and the heating output. Three laboring women, randomly and successively adopting classical birth positions (supine or side positions), sitting or vertical or walking position, were studied.

RESULTS: Fetal heart rate, uterine contractions, ttcpO2 and heating output are influenced by the birth positions and by changes of the birth position. In the classical supine and side position there seemed to be lower fetal oxygenation. Sitting, standing and especially walking were more favorable.

DISCUSSION: Telemetry is useful to study a possible clinical benefit of individual birth positions.
