Etude effectuée à Bombay, en Inde, en 1990, comparant la position accouchée à la liberté de déambulation pendant le 1er stade du travail suivi de la position accroupie pendant le 2ème stade. L’étude porte sur 200 patientes, primigestes et multigestes. 100 ont gardé une position couchée pendant toute la durée du travail et 100 ont marché pendant le premier stade du travail et ont ensuite adopté une position acccroupie pendant le second stade. L’étude montre une réduction de la durée des deux stades du travail pour le groupe accroupi, mais l’incidence des complications est moindre dans le groupe de contrôle (allongé). En conclusion : en l’absence de chaises de naissance qui peuvent apporter un bon support périnéal, la position allongée est préférable dans les conditions de l’étude.

Abstract (English)  :

This study was conducted at the Lokmanya Tilak Municipal General Hospital, Bombay, India during the year 1990. The aim was to compare the routinely used supine position versus ambulation in the first stage and squatting position during the second stage of labour. Our study was comprised of 200 patients both primigravidas and multigravidas; 100 were kept in the supine position throughout labour and 100 were kept ambulatory in the first stage and adopted the squatting position during the second stage. The study showed a shortening of both stages of labour in the squatting group but the incidence of complications was less in the control group. It was concluded that without proper birthing chairs which can give excellent perineal support, the usual supine position is preferable in our setup.

Compare 100 femmes en position allongée pendant tout l’accouchement à 100 femmes laissées libres pendant la première phase du travail puis en position accroupie. On trouve plus de complications dans le groupe accroupi. Remarque : 4 morts périnatales sur 200, épisiotomie systématique pour primigestes dans groupe allongé. Etude prise en compte dans la revue systémtatique Cochrane Gupta 2004 (fiche afar 1099), dans la revue Cochrane de Nikodem (2000) et dans celle de Vendittelli 1998 (Fiche AFAR 2042).
