L’effet de l’accouchement en chaise de naissance sur la durée du travail au deuxième stade, les résultats pour le fœtus et les pertes de sang maternelles ont été examinés dans une étude rétrospective. L’échantillon comprenait 60 femmes primipares, 37 à 41 semaines de gestation avec une grossesse et un travail normaux, 30 accouchements sur une table d’accouchement traditionnelle et 30 accouchements dans un fauteuil d’accouchement. Des comparaisons ont été effectuées entre les groupes sur la durée moyenne du travail au deuxième stade, les scores moyens d’Apgar à une et cinq minutes et les valeurs moyennes d’hémoglobine et d’hématocrite maternelles pendant les périodes pré et post-partum. Aucune différence significative n’a été observée entre les groupes de la table d’accouchement et du tabouret d’accouchement sur la durée moyenne du travail au deuxième stade (chaise d’accouchement, X = 60 minutes par rapport au tableau d’accouchement, X = 43 minutes, t = 1,66, p =.10). Les scores moyens d’Apgar à une et cinq minutes étaient presque identiques. Des différences statistiquement significatives existaient entre les groupes sur les valeurs moyennes d’hémoglobine et d’hématocrite maternelles. L’hémoglobine et l’hématocrite moyenne à l’admission étaient significativement plus élevées dans le groupe du fauteuil de naissance (p inférieur à 0,027). Cependant, après l’accouchement, le groupe de la chaise de naissance avait des valeurs d’hémoglobine et d’hématocrite moyennes significativement plus basses (p inférieur à 0,025). Ces résultats suggèrent que la chaise d’accouchement, en tant que méthode d’accouchement alternative, est sans danger pour le devenir du fœtus mais ne présente aucun avantage pour la mère en termes de travail plus court au deuxième stade. Il est recommandé de poursuivre les recherches sur les pertes de sang maternelles afin d’exclure les effets indésirables éventuels.

Abstract (English)  :

The effect of delivering in a birth chair on duration of second stage labor, fetal outcome, and maternal blood loss was examined in a retrospective study. The sample consisted of 60 primiparous women, 37 to 41 weeks gestation with a normal pregnancy and labor, 30 delivering on a traditional delivery table and 30 delivering in a birth chair. Comparisons were made between groups for mean duration of second stage labor, mean Apgar scores at one and five minutes, and mean maternal hemoglobin and hematocrit values during the pre- and post-partum periods. No significant difference was found between delivery table and birth chair groups for mean duration of second stage labor (birth chair, X = 60 minutes versus delivery table, X = 43 minutes, t = 1.66, p =.10). Mean Apgar scores at one and five minutes were nearly identical. Statistically significant differences existed between groups in mean maternal hemoglobin and hematocrit values. Both the mean hemoglobin and the mean hematocrit upon admission were significantly higher in the birth chair group (p less than.027). However, postpartally the birth chair group had significantly lower mean hemoglobin and hematocrit values (p less than.025). These findings suggest that the birth chair, as an alternate delivery method, is safe in terms of fetal outcome but presents no advantage to the mother in terms of shorter second stage labor. Further investigation of maternal blood loss is recommended to rule out possible untoward effects.
