The conditions under which postpartum haemorrhage becomes a serious bleeding are investigated. Independently identified factors: primiparity; history of postpartum hemorrhage; history of caesarean section; cervical ripening; prolonged work; episiotomy; delay in the initial management of postpartum haemorrhage (ie > 10 minutes or > 20 minutes between diagnosis of bleeding and oxytocin administration); uterine revision (“manual“ examination of the uterine cavity) > 20 minutes later; call of an obstetrician and/or anesthesia > 10 minutes later; delivery in public non-university hospital. Protective effect of the epidural, may be linked to speed of intervention.

L’amniotomie de routine raccourcit la durée du travail mais nécessite davantage de péridurale. Elle augmente la fréquence d’anormalité au niveau de la fréquence cardiaque du foetus.
Routine ARM shorten labour but requires more epidurals. It increases the fetal heart rate abnormalities.

Les résultats de cette étude ne soutiennent pas la croyance que l’amniotomie précoce est une méthode efficace pour réduire la durée du travail.
The results of the study fail to support the long held belief that early amniotomy is an effective method for reducing labour duration.
