Cet essai compare 100 femmes qui ont accouché allongées sur le dos (dans un même lieu d’accouchement) à 98 femmes qui ont accouché en position assise et/ou accroupie et/ou à 4 pattes ou à genoux (dans un autre lieu d’accouchement). Il y a moins de déchirures du périnée et moins d’oedèmes de la vulve, et aussi moins de perte de sang parmi les femmes qui ont accouché en position autre qu’allongée, sans augmentation de risque pour le bébé.

la ventouse est associé à un taux plus bas de lésions du 3e degré que le forceps. L’épisio ne prévient pas toujours le 3e degré. La suture est mal exécutée la plupart du temps chez les femme ayant un 3e degré, source de lésions sphynctériennes et d’incontinence. anale pour la moitié d’entre elles, à cause d’atteintes mécaniques du sphyncter plus qu’en raison de lésions du nerf pudendal. L’attention devrait être portée vers des pratiques obstétricales préventives et les techniques chirurgicales de reconstruction.

Angleterre, étude sur 427 primipares en position demi-assise ou en position accroupie avec « coussin de naissance » pendant la deuxième phase du travail. Moins de forceps, moins de déchirures du périnée mais plus de déchirures des lèvres, durée du second stade plus courte pour la position accroupie, sans différence pour les autres résultats (apgar, pertes sanguines, oedème vulvaire).

Angleterre. Primigestes sans péridurale. Comparaison entre les positions horizontales (latérale ou demi-assise) et le choix libre de positions verticales (genoux, accroupi, assis, debout, selon le choix de la patiente) pendant second stade du travail. Avantage aux positions verticales (en particulier plus de périnée intact ; impression d’accélération en passant de position horizontale à verticale), pas d’effet négatif sur le bébé (Apgar et pH du cordon), pertes de sang identiques. Acceptabilité par sages-femmes et parturientes même sans préparation ni assistance spécifique, surtout pour position à genoux (la position accroupie est difficile à maintenir).
