Chez des femmes obèses (indice de masse corporelle IMC > 35) , un programme d’entretiens diététique (10 fois 1 heure) a permis de réduire la prise de poids pendant la grossesse à 6,6 kg contre 13.3 kg pour le groupe sans entretiens. Ceci s’accompagne d’un meilleur métabolisme glucidique.

Comment mesure la prise de poids pendant la grossesse: nouvelle méthode qui utilise la surface sous la courbe de prise de poids. Cette méthode ne s’avère pas plus efficace que les autres pour prédire le poids de naissance, mais elle est supérieure pour prédire la rétention pondérale à 12, 24 ou 36 mois (mais pas à 6 mois).

Etude danoise sur les risques de bébés trop petits (moins de 3kg) ou trop gros (4 kg et plus) en fonction de la prise de poids maternelle pour chaque catégorie d’indice de masse corporelle (IMC), comparés aux recommandations américaines. Il en résulte que les recommandations américaines peuvent s’appliquer aux femmes de poids normal, en surpoids et obèses ; mais pour les femmes en sous-poids, en comparant les risques de bébés trop petits avec ceux de bébé trop gros, l’étude trouve que la limite de prise de poids maximum devrait être relevée.

Obésité avant grossesse avec prise de poids de plus de 15 kg augmente significativement les risques de: césarienne pour primipares et multipares, macrosomie, et (non-) allaitement à 10 semaines. Par contre, pas de différence entre prises de poids 7-11kg et prise de poids 11-15 kg. Conclusion (pas évident de comprendre d’où elle vient): il vaut mieux informer les femmes avant la grossesse qu’un bon indice de masse corporel est meilleur.

Etude effectuée sur les registres du Missouri, 1999-2000, sur des grossesse non gémellaires avec naissance à terme. Comparées aux femmes qui prennent 11 à 16 kg (prise recommandée par l’Institut de médecine), Les femmes qui prennent moins de 11 kg ont moins de risques de: prééclampsie, disproportions céphalo-pelviennes, échec de déclenchement, césarienne, et macrosomie ; mais plus de risques de "petit bébé" (RCIU? ou "petit pour l’âge gestationnel). Celles qui prennent plus de 16 kg ont: plus de risques de prééclampsie, d’échec du déclenchement, de césarienne et de macrosomie, mais moins de risques d’avoir un petit bébé. L’étude conclut que les recommandations de l’Institut de médecine permettent de bien de minimiser les risques de la grossesse et de l’accouchement, tous risques confondus.

Etude sur large population en Suède. Pour chaque catégorie d’indice de masse corporelle (IMC) (moins de 20, 20-25, 25-30, plus de 30), l’étude trouve à partir de quel gain de poids pendant la grossesse on trouve un accroissement du risque de complications (pééclampsie, hémorragies postpartum, complications veineuses [?veinous complications], dystocie des épaules, complications d’anesthésie, morts-nés après 28 SG, détresse respiratoire du nouveau né, infection bactérienne, problèmes hémorragiques, convulsions, mort périnatale, et score Apgar inférieur à 7 à 5 minutes). Les limites de prise de poids trouvées sont inférieures à celles recommandées actuellement: moins de 10 kg pour tout le monde, moins de 9 kg pour IMC 25-30, moins de 6 kg pour les IMC supérieures à 30.

Cette étude (sur plus de 44 000 femmes au Danemark) a permis d’identifier 3 grand types d’habitudes alimentaires des femmes enceintes: celles qui consomment surtout des viandes rouges, des viandes transformées et des laitages entiers ; celles qui consomment fruits, légumes, volailles et poisson ; celles qui consomment des aliments des deux groupes.
Les femmes qui consomment viandes rouges, transformées et laitages entiers ont un risque accru d’avoir un bébé de poids inférieur à son âge gestationnel.

Liens entre le régime de la mère (proétéines et hydrates de carbones) et certains indicateurs de poids ou indice corporel du bébé, de périmètre crânien, de placenta. Les conclusions ne sont pas évidentes à interpréter au premier abord. Plus la mère prend de protéines, plus les bébés ont un indice pondéral faible. Etude contrôlée sur 132 femmes, 6 maternités réparties en deux groupes. Dans le groupe où des conseils ont été donnés aux femmes sur l’alimentation et l’activité physique, on constate: cela les a aidé à continuer à consommer du pain complet (alors que la consommation décroît dans l’autre groupe), plus de fruits, légumes et fibres. Mais pas d’impact sur le temps d’activité physique de loisirs ni sur la prise de poids pendant la grossesse.

Méta analyse sur 16 études, qui montre que le poids moyen des femmes baisse de façon continue pendant 12 mois après l’accouchement. Mais on a peu de données sur l’évolution après 12 mois. Il semble que les femmes reprennent du poids en moyenne, mais cela serait dû aux modes de vie et non à la grossesse. Les auteurs recommandent de réserver le terme "rétention de poids post partum" à une durée limitée 12-18 mois après l’accouchement.

Cette étude (Suède) prouve, au vu des complications, qu’il y a un avantage à perdre du poids avant la grossesse pour les femmes obèses et en surpoidsj. Elle suggère que les femmes de poids normal auraient aussi intérêt à ne pas prendre de poids (même si elles restent dans les poids normaux)

Etude sur femmes de poids normal ou bas qui confirme un lien entre faible prise de poids (moins 0.27 kg par semaine) et accouchement prématuré. Mais des différences selon le type racial, et selon les précédents:

Pour les afro-américaines avec antécédent de prématurité uniquement, une prise de poids forte est, comme une prise de poids faible, associée à la prématurité. Cela ne se retrouve pas dans les autres groupes.

Pour les asiatiques avec antécédant de prématurité, une faible prise de poids n’est pas associée à un nouvel accouchement prématuré.

complications associées à une prise de poids supérieures à celle recommandée par l’institut de médecine américain: Apgar bas à 5 minutes, convulsions [seizure, vérifier la traduction], hypglycémie, [polycythemia], syndrome d’aspiration méconiale [meconium aspiration syndrome, vérifier traduction], et bébé trop gros par rapport à son âge gestationnel. Pour prise de poids extrêmes (> 18kg), et il a en plus une augmentation des risques d’assistance respiratoire.

Une prise de poids inférieure aux recommandations est associée à un seul résultat négatif: faible poids de naissance. Une prise de poids très faible (< 7kg) est en plus associée à plus de risques de : convulsions [seizure?] et séjour en maternité de plus de 5 jours.

Etats Unis. On évalue les risques de: bébé plus de 4,5kg, bébé en hypoglycémie et en [hyper bilirubinemie] (qui doit correspondre à une jaunisse ou ictère, sévère?) pour les femmes qui ont pris un poids supérieur ou inférieur à celui recommandé par l’institut de médecine américain. Les femmes qui ont pris au dessus des recommandations ont plus de chances d’avoir bébé gros (3 fois plus), hypoglycémie ou jaunisse (1 fois et demie plus). Celles qui ont pris moins que les recommandations ont moins de risque de gros bébé, mais mêmes risques d’hypglycémie ou jaunisse.

Valable pour tout le monde (quel que soit indice de masse corporelle IMC et prise de poids forte/faible). Plus on est grosse avant grossesse (IMC), plus on a de risques de prendre trop de poids. Si on prend moins de poids que recommandé, on fait des bébés plus petits et la durée de gestation est plus courte.

Etude sur 1100 personnes. 53 ont prise trop de poids pendant la grossesse (par rapport aux recommandations de l’Institut de médecine américain). Les contributeurs principaux à cette prise de poids sont les facteurs présents avant la grossesse (non détaillés dans le résumé. On peut suppose indice de masse corporelle, habitudes alimentaires?), alors que les facteurs propres au déroulement de la grossesse et les facteurs qu’on peut modifier pendant la grossesse (alimentation?) ont peu d’influence. Donc, les mesures pour limiter la prise de poids devraient débuter avant la grossesse.

Pour les prises de poids trop faibles par rapport aux même recommandations, il semble qu’on puisse agir avant et pendant la grossesse.
