The conditions under which postpartum haemorrhage becomes a serious bleeding are investigated. Independently identified factors: primiparity; history of postpartum hemorrhage; history of caesarean section; cervical ripening; prolonged work; episiotomy; delay in the initial management of postpartum haemorrhage (ie > 10 minutes or > 20 minutes between diagnosis of bleeding and oxytocin administration); uterine revision (“manual“ examination of the uterine cavity) > 20 minutes later; call of an obstetrician and/or anesthesia > 10 minutes later; delivery in public non-university hospital. Protective effect of the epidural, may be linked to speed of intervention.

High quality meta-analysis comparing supine with other positions for the second stage of labor. The only significant results are, supine vs others : more instrumental deliveries and more episiotomies, more pain and less satisfaction. None of the foetal outcomes shows any significant difference.
